ID Cards

Member App for downloading ID cards

MetLife ID Cards are not required for service. Providers can look up with SSN/DOB. Employees can register on Mybenefits  and download an I.D. Card. For PPO coverage I.D. cards are not required; however, there is an option in MetLink to order small batches of generic I.D. cards and they go directly to the customers address ATTN: to the Benefit Administrator. They can then share with employees that insist on having an I.D. card. For DHMO cards, the personalized cards are sent directly to the member’s home because they are required to select a provider, and this information will be on the personalized card.

Can a broker download ID cards?

No.

Do dependents receive ID cards?

No, the dependents do not receive a different I.D. card unless they have DHMO and select a different provider.
