Can a broker download ID cards?
No, brokers cannot download ID cards.
Do dependents receive ID cards?
Yes, if the dependent is a spouse, they will receive two identical ID cards.
a. Subscribers and covered dependents with the same addresses will be on the same card (up to 5)
b. If the total number of members is more than 5, then the additional covered dependents will be on a second card with the subscriber listed again
c. If one of the covered dependents is a spouse and the spouse’s address is the same, the subscriber will get two cards
Prior Carrier Deductible Credit
Prior Carrier Deductible Credit is given for all plan types (HMO, PPO, Trust).
Prior Carrier Deductible Credit/OOP Max may be credited if valid EOB from prior carrier is submitted within 90 days of the effective date.
This information should be faxed to 866-474-4040 no later than 90 days after the effective date.

New Hire Rating Rules
New hire rates will be based on the member’s age at the member’s enrollment date.
Newborn Coverage
What is the effective date of coverage for a newborn, and how are premiums requested?
The newborn will be covered starting from the birth date, and the group has 31 days to request this from Aetna. Aetna does not pro rate so premiums will start the 1st of the next policy month unless the date of birth is the 1st of the month.
Is there an additional cost for covering the newborn during the first 30 days?
The member’s premium will be charged as EE + Child(ren) or EE + Family.
Does this apply to both the mother and the father?
Yes, this applies to both the mother and father, and the billing process is the same for both Small Group and Large Group.
