Prior Carrier Deductible Credit
Prior carrier deductible and out-of-pocket maximum (OOPM) credits may be considered when a valid Explanation of Benefits (EOB) is submitted within 60 days of the effective date.
Deductible credit is available for PPO-to-PPO and HSA-to-HSA transitions only; HMO plans are not eligible.
No deductible credit form is required. Prior carrier EOBs must be submitted and are subject to review and approval. For groups joining CalCPA Health mid-year, employers may submit a payment report with applicable accumulator information, or members may submit an Accumulator Credit Request Form.
