Prior Carrier Deductible Credit
Prior Carrier Deductible Credit and Out-of-Pocket Maximum (OOPM) credit may be applied when a valid EOB from the prior carrier is submitted within 60 days of implementation.
Deductible credit is available for PPO-to-PPO and HSA-to-HSA plans. HMO plans are not eligible for deductible credit.
No prior carrier deductible credit form is required; copies of prior carrier EOBs must be subitted.
