Newborn Coverage
What is the effective date of coverage for a newborn, and how are premiums requested?
Health Net will enroll newborns as of the date of birth, and the 1st month will be prorated.
Is there an additional cost for covering the newborn during the first 30 days?
Health Net could prorate the premium for the newborn based on the date of birth.
Does this apply to both the mother and the father?
Yes.
ID Cards
Member App for downloading ID cards
Members can pull digital ID cards from the member portal on this site www.healthnet.com, select the Member Log In option, and Employer Plans member registration.
Member Web access instructions on how to access online account

Can a broker download ID cards?
No, but the Account Management team can provide them as needed. You can request this by emailing your Account Manager or calling 800.447.8812.
Do dependents receive ID cards?
Yes.
Prior Carrier Deductible Credit
Prior Carrier Deductible Credit is available for Small Business Group PPO plans with deductibles when replacing a similar PPO plan. CommunityCare HMO plans with deductibles may also qualify.
Prior Carrier Deductible Credit does not include Out-of-Pocket Maximum (OOPM) carryover.
No Prior Carrier Deductible Credit form is required. EOBs, claim ledgers, or deductible credit letters from the prior carrier will be accepted.

Medicare Part D Creditable Coverage (2026)
